The Foundation of Community Hospice & Palliative Care generates philanthropic and community support to help the nearly 1,600 patients and their families who are served daily by Community Hospice & Palliative Care ® to live better with advanced illness.

Contributions go directly to meet the unfunded and underfunded patient care needs of adults and children in the communities we serve. Services include Community PēdsCare®, a nationally recognized pediatric palliative and hospice program, as well as professional education and community outreach programs that help people understand advanced illness and have access to our services.

Your Gift Supports

Hospice Care

Your donation gives the gift of compassionate hospice care in any of Community Hospice & Palliative Care’s 16 counties to patients who do not qualify for Medicare or Medicaid and/or do not have insurance to pay for their end-of-life needs. We feel it is our responsibility to provide this type of charity care.

Your gift changes the life of a patient, their loved ones, and our community forever.

  • 8,591: Number of hospice patients cared for in Fiscal Year 2022.

  • $211*: Your gift funds one day of hospice care at home for a patient within the last sixty days of their life (Routine Home Care).

  • $1,111*: Your gift provides one day of care for patients in one of our centers for caring with the goal of stabilizing their acute pain and symptoms before returning them to the comfort of their home (General Inpatient Care).

  • $1,522*: Your gift funds one day of compassionate care for patients who choose to remain at home and receive around-the-clock care in the final hours of their life (Continuous Home Care).

  • $1,472,639.78: Annual funding need to provide charity hospice care in Fiscal Year 2022.

Pediatric Care The Foundation of Community Hospice and Palliative Care

Pediatric Care

Your gift ensures parents of children, prenatal to age 21, never receive a bill from Community PēdsCare (Community Hospice & Palliative Care’s pediatric program). The program, the only one of its kind in North Florida, provides life-enhancing wraparound care services to pediatric patients in partnership with their physicians and area health systems. Your gift supports the emotional, physical, and psychological needs of children and their families living with impact of life-limiting and life-ending illnesses.

  • $502: Your gift funds one month of Community PēdsCare services for a child and their family.

  • $10,000: Your support provides one year of Community PedsCare services for a child and their family.

  • $1,403,670.64: Annual funding need to provide the Community PēdsCare program in Fiscal Year 2022.

Grief Support

Your gift funds grief support for the people of the community whose loved one did not die in the care of Community Hospice & Palliative Care. Every month, Community Hospice & Palliative Care hosts “Understanding Grief” support sessions both in-person and online throughout their service area. Your gift provides people with the tools, techniques, and support they need to grieve the loss of someone they loved.

Veteran Support The Foundation of Community Hopice and Palliative Care

Veterans

Your donation funds one month of honor ceremonies for veteran patients in hospice care. Thanks to you, every veteran will be honored with an “Honored Veteran” pin and branch of service certificate acknowledging their selfless contribution to our nation.

Your gift ensures veteran patients are honored for their service and sacrifices before they die.
