The Foundation of Community Hospice & Palliative Care generates philanthropic and community support to help the nearly 1,600 patients and their families who are served daily by Community Hospice & Palliative Care to live better with advanced illness.

Contributions go directly to meet the unfunded and underfunded patient care needs of adults and children in the communities we serve. Services include Community PedsCare, a nationally recognized pediatric palliative and hospice program, as well as professional education and community outreach programs that help ensure people understand advanced illness and have access to our services.
